- Must be 18-years old to participate.
- Registration & Payment for class must be made prior to the class start.
- Clients must cancel at least 24 hours in advance. Cancellations less than 24 hours, as well as no-shows, will not be eligible for a refund.
- Class schedule is subject to change or be cancelled due to low attendance or unforeseen circumstances. In this event, instructor will refund payment.
- The liability release must be acknowledged to participate.
- These are fitness classes, not technical dance training. These classes are beginner-friendly. The goal is to move to great music and have fun!
- Dance shoes are preferred. Sneakers, socks, and bare feet are accepted. If wearing shoes for Heels class, they must provide ankle support! If the shoes worn for Heels class do not provide ankle support, you will be asked to remove them.
- I do not sensor music selections and cannot be held responsible if you are offended. While I do opt for “clean” versions of audio tracks, this is not always available or a guarantee.
LIABILITY RELEASE
In signing the line below I agree that Sculpt Pilates and Barre and Bring It To The Dance Floor is in no way responsible for the safekeeping of my personal belongings while I attend class. I understand that classes at Sculpt Pilates and Barre and Bring It To The Dance Floor may be physically strenuous and I voluntarily participate in them with full knowledge that there is risk of personal injury, property loss, or death. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against Sculpt Pilates and Barre or Bring It To The Dance Floor or its members for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise.
PHOTO, MEDIA + COPYRIGHT RELEASE
I grant Sculpt Pilates and Barre and Bring It To The Dancefloor permission to photograph and/or videotape myself during in-studio/live classes and/or privates at Sculpt Pilates and Barre and Bring It To The Dance Floor. These photographs/videos will remain property of Sculpt Pilates and Barre and Bring It To The Dance Floor and may be used in advertising or marketing campaigns on Sculpt Pilates and Barre’s or Bring It To The Dance Floor’s website, and for promotional and informational material including, but not limited to, flyers, brochures, emails, advertisements, newspaper articles, TV, or cable interviews/promotions. I understand I will not be identified by name unless I give my permission, I hereby waive and release any rights to compensation for, or ownership of, such images.
ASSUMPTION OF THE RISK & WAIVER OF LIABILITY RELATING TO CORONAVIRUS/COVID-19
In accordance with the most recent guidance and protocols issued by the World Health Association (WHO) and the Centers for Disease Control and Prevention (CDC), the attendee hereby agrees, represents, and warrants that the undersigned shall not visit or utilize facilities, services, programs of Sculpt Pilates and Barre and Bring It To The Dance Floor (other than exclusively online) within 14 days after (i) returning from highly impacted areas subject to a CDC Level 3 Travel Health Notice, (ii) exposure to any person returning from areas subject to a CDC Level 3 Travel Health Notice, or (iii) exposure to any person who has a suspected or confirmed case of COVID-19. The undersigned hereby agrees, represents, and warrants that the undersigned shall not utilize the facilities, services, and programs of Sculpt Pilates and Barre and Bring It To The Dance Floor if he or she (i) experiences symptoms of COVID-19, (ii) has a suspected or diagnosed/confirmed case of COVID-19. Sculpt Pilates and Barre and Bring It To The Dance Floor has implemented the recommended protocols and preventative measures recommended by the CDC to help reduce the spread of COVID-19; however, Sculpt Pilates and Barre and Bring It To The Dance Floor cannot guarantee that you will not become infected with COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending Sculpt Pilates and Barre and Bring It To The Dance Floor and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at Sculpt Pilates and Barre and and Bring It To The Dance Floor may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Sculpt Pilates and Barre and Bring It To The Dance Floor instructors, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at Sculpt Pilates and Barre and Bring It To The Dance Floor or participation in Sculpt Pilates and Barre and Bring It To The Dance Floor classes. On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless Sculpt Pilates and Barre and Bring It To The Dance Floor, its instructors, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Sculpt Pilates and Barre and Bring It To The Dance Floor, its instructors, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any program with Sculpt Pilates and Barre and Bring It To The Dance Floor.
ACKNOWLEDGMENT REQUIRED TO REGISTER
The registered attendee acknowledges and agrees to comply with Sculpt Pilates and Barre’s and Bring It To The Dance Floor’s procedures at all times. By clicking accept, the registered attendee signifies they have understood the terms of this assumption of risk, release and waiver of liability, and indemnity agreement and agree to its terms.
